I don't put the cap on the end of the pen while I write - it's super heavy and I find it fatiguing; I've had to replace one nib due to dropping the pen - that cost just under $200 - if you put the cap on the end, your's won't roll off the table like mine did.

The screw on cap is fat and heavy and does not present a streamlined plane at one with the barrel; once the cap is off, the grip fits nicely in my small hand.
